Description

Job Title: Licensed Master Social Worker (LMSW) – Home Health

Location: Portland, OR

Position Type: Contract (13 Weeks)

Pay Rate: $42–$52/hour

Shift & Schedule:

  • Day Shift

  • Monday–Friday

  • 8:00 AM – 4:30 PM

  • 5 × 8-hour shifts

  • 40 hours per week

  • No weekends

  • No on-call

Position Overview:
We are seeking a compassionate and experienced Licensed Master Social Worker (LMSW) to provide home health services to patients throughout the Portland area. The ideal candidate will have skilled home health experience, strong OASIS documentation knowledge, and a passion for delivering patient-centered care.

Responsibilities:

  • Conduct psychosocial assessments for home health patients.

  • Develop and implement individualized care plans.

  • Coordinate community resources and discharge planning.

  • Provide counseling and emotional support to patients and families.

  • Complete accurate OASIS and EPIC documentation.

  • Collaborate with physicians, nurses, therapists, and the interdisciplinary healthcare team.

  • Travel to patient homes while maintaining productivity standards.

  • Educate patients and families on available community resources and support services.

Requirements:

  • Active LMSW License (or equivalent state licensure)

  • Minimum 1 year of skilled Home Health experience

  • OASIS documentation experience

  • Current BCLS Certification

  • Valid driver's license, reliable transportation, and auto insurance

  • COVID-19 primary vaccination series plus one booster

  • Strong communication, documentation, and patient advocacy skills

  • First-time travelers are welcome

  • Pending Oregon license accepted
