POSITION SUMMARY:

Saint James Health Community Health Center (SJH) is seeking a Licensed Clinical Social Worker (LCSW). SJH provides preventive, primary care, and wellness services at multiple locations in Newark, NJ.

The LCSW is responsible for supporting social work services and implementing care plans in coordination with medical treatment. The LCSW assesses patient needs, develops treatment plans, and provides interventions to support patient care. This role works in collaboration with clinical staff to ensure coordination of services and continuity of care.

RESPONSIBILITIES:

  • Provide social services to support the physical, social, and psychological functioning of patients
  • Assess psychosocial status, support systems, and available resources to develop appropriate care plans
  • Conduct psychosocial assessments and provide interventions to assist patients in managing medical and life circumstances
  • Identify emotional or behavioral conditions and document diagnoses in accordance with ICD-10 guidelines
  • Provide individual and group counseling, as applicable
  • Develop and implement treatment plans, including type, frequency, and duration of services
  • Coordinate services with internal departments, community agencies, and external providers
  • Maintain working knowledge of community resources and support service coordination
  • Participate in care planning with clinical teams and support coordination across the continuum of care
  • Maintain documentation in the electronic medical record in accordance with policies
  • Participate in meetings, committees, and required organizational activities
  • Maintain knowledge of relevant medical and legal considerations impacting patient care (e.g., advance directives, mandated reporting)
  • Support operational efficiency by participating in team initiatives and process improvements
  • Complete required trainings and maintain compliance with organizational policies
  • Track and maintain assigned work in designated systems, ensuring accuracy and accessibility
  • Perform other related duties as assigned

REQUIREMENTS:

  • Bilingual (Spanish or Portuguese) preferred

EDUCATION AND EXPERIENCE:

  • Bachelor's degree in a related field required
  • Master's degree in Social Work required
  • Minimum of three (3) years of experience as a social worker in a healthcare setting, medical center, or related environment

LICENSURE:

  • Active New Jersey Licensed Clinical Social Worker (LCSW) license required
