Responsibilities ----------------

Provide behavioral health counseling services.

• Conduct patient assessments and establish therapy treatment goals
• Administer individual and group therapy
• Maintain thorough and confidential records of observations
• Evaluate progress and effectiveness of therapy interventions
• Provide patient with information regarding resources and therapeutic activities in the community
• Work directly with the patient, family, physician and community providers to ensure active and collaborative care from the initiation of treatment to the time of discharge

Requirements ------------

Licensure / Certification / Registration:

• One or more of the following required:
• Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy
• Professional Counseling & Social Work obtained prior to hire date or job transfer date
• Social Worker specializing in Clinical Practice credentialed from the Wisconsin Board of Marriage & Family Therapy
• Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy
• Therapist specializing in Marriage & Family credentialed from the Wisconsin Board of Marriage & Family Therapy
• BLS Provider preferred. American Heart Association or American Red Cross accepted.
• Supervisor credentialed from the Wisconsin Department of Safety and Professional Services preferred.

Education:

• High School diploma equivalency with 2 years of cumulative experience OR Associates degree/Technical degree OR 4 years of applicable cumulative job specific experience required.
• Additional Preferences: Masters Degree in Counseling, Social Work or equivalent required.
• Active license in the state of WI required: LPC-IT, LPC, APSW, LCSW or equivalent.

Acute care behavioral health experience preferred within a PHP or IOP.
