POSITION SUMMARY

The CalAIM Lead will play a pivotal role in ensuring the success of the CalAIM program. This position is

responsible for assisting Enhanced Care Management (ECM) Case Managers and Housing Navigators,

reviewing ECM and Community Support referrals, revising care plans and housing plans, tracking staff

productivity and client engagement. The Lead will provide leadership and management to uphold the

mission and core values of the organization, while also contributing to the development and improvement of

operational practices.

ESSENTIAL DUTIES AND RESPONSIBILITIES

The essential functions include, but are not limited to the following:

Provide guidance and support to ECM Case Managers and Housing Navigators to ensure efficient and

effective service delivery.

Lead by example to maintain a positive and collaborative work environment.

Address team concerns and provide solutions to enhance productivity and morale.

Review and approve ECM and Community Support referrals for accuracy and compliance.

Oversee the revision and finalization of care plans and housing plans to meet client needs and program

standards.

Monitor program performance metrics and provide regular reports to the Program Manager.

Effectively communicate policies, updates, and operational changes to team members.

Act as a liaison between the Program Manager and team, ensuring alignment with program goals.

Collaborate with other departments to ensure seamless delivery of services.

Support onboarding and training of new team members.

Provide ongoing coaching and mentorship to staff to encourage professional growth.

MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)

Knowledge of the Fresno Madera Continuum of Care and Housing Services

2-3 years of supervisory experience in a social services, healthcare, or housing-focused program.

2-3 years of experience in CalAIM, ECM, and Community Support services.

Demonstrated ability to manage multiple priorities and meet deadlines.

Exceptional problem-solving, communication, and organizational skills.

Proficiency in Microsoft Office Suite and program management software.

Cleaning driving record
