I.  Position Summary

Reporting to the Senior Director of Nursing of Hebrew Rehabilitation Center (HRC), the Social Service Leader is responsible for overseeing the integration culture change for Social Service staff and for oversight of social service practice including clinical supervision.

 

II.  Core Competencies:

  • Provide excellent leadership and supervision to staff

  • Communicate effectively to staff, senior management, patients and families

  • Enhance processes and systems to improve the delivery of patient centered care

  • Provide leadership for the integration of culture change with the social service staff

 

III. Position Responsibilities:

  • Responsible for the hiring, training, and evaluation of social work staff in collaboration with the other health care team members.

  • Provides leadership for initiatives that directly involve social service ie Advance Directives, Concern Resolution, Guardianship, Admissions and Discharge Planning.

  • Clinical supervision for social workers on LTCH units at HRC and social work interns.

  • Key member of interdisciplinary team responsible for the assessment and care planning process of patients and culture change initiatives.

  • Oversight of department communications with patient’s family members that provide updates on patient well-being, address family concerns, orient families to HRC at admission, assist in the patient’s psychosocial adjustment to HRC, and coordinate discharge planning as needed.

  • Carry patient caseload as identified.

  • Coordinates student internship programs including oversight of school contracts and student requirements.

  • Assists with review of social service continuing education credits as requested.

  • Participation in a variety of organizational committees and initiatives, including Family Advisory Council.

  • Maintain compliance with all federal and state regulations and assist in federal and state audits as necessary

 

IV.  Qualifications:

  • Must have current LICSW

  • A minimum of  2-5 years supervisory experience in a LTC or sub-acute setting preferred. 

  • Experience with staff management and superior customer service and communication skills required.

 

V.  Physical Requirements:

·   Ability to manage prolonged, extensive or considerable standing or walking.

·   Ability to manage some transporting of patients and moving of equipment (e.g., wheelchair).

·   Ability to manage some stooping, bending and crouching.

·   Ability to manage contact with patients under a variety of circumstances, which may include exposure to unpleasant odors, sights and sounds as well as possible exposure to infections, contagious diseases and blood borne pathogens.
