Curant Health, the leader in patient-centric chronic disease management via specialty pharmacy is searching for a Lead Patient Care Coordinator to join its team in St. Petersburg, FL. We are committed to improving patient health outcomes by offering a pharmacist-managed medication care program, comprehensive therapeutic monitoring and dependable home delivery of prescription drugs.

This position is primarily responsible for managing the prescription fulfillment process from inception to delivery by compiling and maintaining patient data to document patient’s prescription profile, conditions and treatment. This position serves as a link between patient, physician and pharmacy.  The Lead PCC is also responsible for training within the PCC department and preparation of a variety of reports as assigned by their supervisor.

Responsibilities

  • Provide a superior patient experience by providing pharmacy customer service to an assigned group of patients.
  • Make outgoing refill reminder calls to patients with chronic diseases.
  • Provides training support to new and existing PCC staff.
  • Monitors service calls to observe employee demeanor, technical accuracy, and conformity to company policies for training purposes.
  • Answers questions and recommends corrective services to address customer issues.
  • Develops quick reference guides and other reference materials for PCCs as needed
  • Accurately input necessary data to keep patient records up to date

Qualifications

  • High school diploma or equivalent
  • 2 years PCC experience
  • Pharmacy Technician Registration
