The Women and Children's Services Lead Clinical Social Worker, LCSW, at Cedars-Sinai in Los Angeles, California, is the Social Work teams point person for first-level escalated patient care consultations. The Lead will assist in identifying, developing, planning, implementing, and evaluating all psychosocial and social service-related interventions. The Lead develops and collaborates with the care team on patient and family care goals and treatment plans. The Lead will assist the team in continuously improving Social Work processes, taking the lead on training plans for new team members, assisting with periodic Documentation Audits and additional projects/tasks as necessary.
Whats this role all about?
Provide first-level escalation support for the Women and Children Services Team. Develop patient and family care goals and treatment plans as agreed upon by the patients care team. Is available to physicians, nurses, and care management team as a consultant for those patients with complex psychosocial needs. Utilize clinical judgment in making recommendations that will assist in addressing psychosocial barriers to treatment and discharge planning, coordinates discharge to post-acute care resources as needed for often complex cases and under-resourced patients. Assist in training newly hired employees. Provide back-up support for staffing shortages. Participate in annual Audits. Responsible for the development, planning, implementation, and evaluation of all social service-related interventions. Attends team rounds and participates in interdisciplinary collaborations.
Qualifications
Educational Requirements: Masters Degree in Social Work
License/Certification/Registration Requirements: Completed LCSW Licensure Completed CCS (California Childrens Services)-Paneled Provider - strongly preferred

5-8 years of Social Work experience, of which 5 years is focused on Women's and Children's Services in an acute care hospital or healthcare Social Work setting Lead or supervisor experience strongly preferred
