This is a hybrid position (2 days remote, 3 days in the field)<\/b>, giving you the flexibility to work from home while building strong, in -person connections with members across High Desert regions.

Key Responsibilities<\/b><\/span>
<\/h3>

• Serve as the primary point of contact<\/b> for members and coordinate their care
  <\/li>
• Develop and manage personalized care plans<\/b> with members and providers
  <\/li>
• Collaborate with healthcare teams and community resources
  <\/li>
• Support members through education, advocacy, and engagement<\/b>
  <\/li>
• Identify and address gaps in care
  <\/li>
• Maintain accurate, timely documentation
  <\/li><\/ul><\/div><\/span>
  

  Requirements<\/h3>

  • Experience in care or case management<\/b> (community -based preferred)
    <\/span><\/li>
  • Knowledge of Medi -Cal and ECM<\/b> strongly preferred
    <\/li>
  • Strong communication and interpersonal skills
    <\/li>
  • Ability to work in a team -oriented environment<\/b>
    <\/li>
  • Willingness to travel within Orange County<\/b>
    <\/li>
  • Bilingual (English/Spanish) preferred<\/b>
