Pendleton Transition CoordinatorThe Transition Specialist for Indiana's Department of Correction: Division of Youth Services provides oversight to all transition-related practices in the State's juvenile correctional facilities.Position is on-site, Monday thru Friday. This position requires the ability to successfully pass CAI and DOC background checks including a drug screen, TB test, DCS check and fingerprinting. Position is located in a juvenile correctional facility and may be required to supervise students in the classroom in the absence of the classroom teacher.Job Responsibilities may include, but not limited to:Facilitate the implementation of a life skills curriculum, titled Next Steps.Utilize the Transition Coordinator Handbook in an effort to standardize the transition practices.Evaluate the five "Transition Consultations" to be completed by every student under the guidance of an assigned teacher.Participate on a weekly basis in Administrative Release Committees (ARC) in each facility to confirm and reinforce the student's reentry plans.Do the Follow Up Phone Calls to released youth at 1, 3, 6, 9, and 12 months.The Transition Coordinators will represent the Indiana Department of Correction- Division of Youth Services at many conferences and meetings.The Transition Coordinators will assist in the development of a partnership with Indiana's Department of Workforce Development, HIRE, and Indiana University’s HOPE Mentoring.The Transition Coordinators will assist in the development of the individualized Education Learning Plan for each youth upon entry at the facility.Work with state, county and private agencies to provide re-entry servicers for youth transitioning back into the community.Assist students with obtaining social security cards, state identification, driver’s license, birth certificates, and additional documentation.Assist students with transitioning back to public schoolsAssist students with employment, apprenticeship programs, and military service informationRequires skills:Ability to communicate effectively in a team environment.Experience with training, coaching or guiding youth.Experience collaborating and creating initiatives.Experience as a spokesperson at events.Experience with building relationships and partnerships to leverage resources and services.
