American Senior Communities is now hiring a ISNP Coordinator
This position will cover the communities in Southern Indiana
The ISNP (Institutional Special Needs Plan) Coordinator is responsible for establishing and nurturing relationships with Senior Living Facilities and Skilled Nursing Facilities to introduce the affiliation with Provider Partners ISNP and IESNP (Institutional Equivalent Specials Needs Plan). This position is also responsible for identifying eligible residents, gathering essential information, reaching out to the residents or responsible parties to make them aware of the affiliation with Provider Partners and offer the eligible resident or responsible party the opportunity to meet with a Provider Partners Benefit Advisor.
Essential Position functions
- Develop and maintain relationships with key contacts at participating facilities.
- Identify residents who meet plan eligibility requirements.
- Outreach to residents or responsible parties by person-to-person phone calls to present the affiliation information and offer the opportunity to meet with a Provider Partners Benefit Advisor.
- Ensure all activities comply with relevant regulations and industry standards, especially in healthcare-related services.
- Complete and return consent to contact forms (lead cards) to Provider Partners Health Plan.
- Maintains documentation on calls, feedback, and outcomes.
- Monitors census monthly to identify newly eligible residents.
- Provide education to facility leaders and interdisciplinary teams related to the ISNP plan benefits and eligibility requirements.
- Host ISNP Open House events for residents and responsible parties and various facilities.
- Maintains specific knowledge and updates related to the ISNP plan.
- Communicates enrollment status to Senior Vice President of Field Accounting and ASC Leadership.
- Completes other special projects as assigned.
Education and Qualifications
Previous experience (min 4 years preferred) in long-term care or assisted living.
Proficiency in Excel.
Previous experience (min 4 years preferred) in lead generation, business development, or a related role
Ability to travel for on-site facility visits.
