Community Behavioral Health (CBH) is a multi-site outpatient mental health organization serving children, adolescents, and adults across Maryland. We provide comprehensive behavioral health services — including therapy, psychiatry, PRP, IOP, and Behavioral Health Home coordination — to promote recovery, stability, and holistic well-being.

Position Summary

We are seeking a Registered Nurse (RN) to join our interdisciplinary team as an IOP / BHH Nurse.

This full-time role combines Intensive Outpatient Program (IOP) nursing in the mornings and Behavioral Health Home (BHH) care coordination in the afternoons (40 hours/week).

The ideal candidate is a compassionate and organized RN who enjoys both direct patient care and care coordination across behavioral and physical health needs.

Schedule

  • Full-time, 40 hours/week

  • Mornings: On-site at IOP Program (child/adolescent population)

  • Afternoons: Health Home care coordination (primarily adult caseload)

Key Responsibilities

Intensive Outpatient Program (IOP) – Morning

  • Facilitate health education and wellness groups for children/adolescents.

  • Support milieu management and collaborate with therapists and PRP staff.

  • Complete authorizations, intake coordination, and family communication.

  • Monitor attendance and notify the clinical team of absences or safety concerns.

  • Participate in treatment team meetings and ensure compliance with COMAR.

Behavioral Health Home (BHH) – Afternoon

  • Conduct nursing assessments for enrolled Health Home participants under COMAR 10.09.79.

  • Develop and update Health Action Plans with patients and multidisciplinary teams.

  • Coordinate medical and behavioral health services, including hospital transitions and specialist referrals.

  • Administer medications and injections; provide patient/family education on medication management.

  • Track preventive care (labs, vitals, screenings) and follow up on abnormal results.

  • Promote chronic disease management and wellness education (nutrition, exercise, medication adherence).

  • Link patients to community and social support services to reduce barriers to care.

Qualifications

  • Required: Active Maryland RN license in good standing.

  • Preferred: 2+ years in behavioral health, psychiatry, or care coordination.

  • Skills:

    • Strong communication and documentation skills.

    • Comfort with electronic medical records (EMR).

    • Knowledge of COMAR Health Home requirements preferred.

    • Compassionate, reliable, and team-oriented.
