The Intensive Community Therapist provides intensive, community-based behavioral health services to children, adolescents, adults, and families experiencing significant emotional, behavioral, or mental health challenges. Services are delivered in homes, schools, community settings, and other natural environments to promote stabilization, improve functioning, and prevent hospitalization or out-of-home placement.

The Intensive Community Therapist provides intensive, community-based behavioral health services to children, adolescents, adults, and families experiencing significant emotional, behavioral, or mental health challenges. Services are delivered in homes, schools, community settings, and other natural environments to promote stabilization, improve functioning, and prevent hospitalization or out-of-home placement. The therapist collaborates with clients, families, community providers, and interdisciplinary team members to develop and implement individualized treatment plans.


Essential Duties and Responsibilities

• Conduct comprehensive behavioral health and psychosocial assessments.
• Develop, implement, and regularly update individualized treatment plans based on client strengths and identified needs.
• Provide evidence-based individual, family, and crisis intervention services in community settings.
• Deliver intensive therapeutic services designed to improve emotional regulation, coping skills, communication, and overall functioning.
• Collaborate with schools, healthcare providers, social service agencies, probation officers, and other community partners to coordinate care.
• Participate in crisis response, safety planning, and risk assessments as needed.
• Maintain accurate, timely, and complete clinical documentation in accordance with agency, state, and payer requirements.
• Monitor client progress using measurable treatment goals and adjust interventions as appropriate.
• Educate clients and families regarding mental health conditions, coping strategies, and available community resources.
• Participate in clinical supervision, case consultations, staff meetings, and required training.
• Maintain compliance with confidentiality regulations, ethical standards, and agency policies.
• Participate in an on-call rotation if required.
