SCOPE OF ROLE
Under the supervision of the Ryan White Program Manager, the Intensive Behavioral Health Medical Case Manager provides a range of client-centered services that link clients with medical, psychosocial, and other needed services. The coordination and follow-up of medical and behavioral health treatments is a key component of intensive behavioral health medical case management. These services ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care through ongoing assessment of the client's and client's key family members' needs and personal support systems. Client and family encounters may include face-to-face and phone contact as well as other forms of communication. Visits will be office based and/or home and/or community based per the individualized care plan. The goal of the Intensive Behavioral Health Medical Case Manager is to focus more intensively on behavioral health issues of Ryan White clients in order to address barriers to consistent medical care and medication management which may lead to greater likelihood of viral load suppression.
