About the Job:

The Intake Support Specialist plays a critical role in coordinating patient referrals and ensuring a smooth intake process for home health and hospice services. This position serves as a key point of contact for referral sources, patients, and internal teams, ensuring accurate information, timely processing, and excellent customer service.

What You’ll Do:

  • Manage incoming referrals and ensure accurate, timely processing
  • Educate callers on agency services and connect them to appropriate resources
  • Verify insurance benefits, authorizations, and payer requirements
  • Create and maintain patient records in the EMR system
  • Identify missing documentation and follow up with referral sources
  • Collaborate with care teams, liaisons, and departments to coordinate admissions
  • Route referrals appropriately to support timely start of care
  • Deliver exceptional customer service to internal and external partners
  • Maintain productivity standards and contribute to a team-oriented environment

Who You Are:

  • Detail-oriented with strong organizational and critical thinking skills
  • Effective communicator (written and verbal) with a customer-first mindset
  • Collaborative team player with a positive, solution-focused attitude
  • Comfortable managing multiple tasks in a fast-paced environment
  • High school diploma required; healthcare or medical office experience preferred
  • Familiarity with EMR systems and insurance verification is a plus
  • Meets background check requirements
