Intake Coordinator (RN/LPN Preferred) – Home Health & Hospice

Position Summary

KeystoneCare is seeking an experienced and compassionate Intake Coordinator to join our Home Health and Hospice team. This critical role serves as the primary liaison between hospitals, physicians, discharge planners, referral sources, patients, and clinical teams to ensure a seamless transition of care. The ideal candidate will have a strong clinical understanding of home health and hospice services and experience coordinating patient admissions in a fast-paced healthcare environment.

Preference will be given to candidates who are licensed Registered Nurses (RN) or Licensed Practical Nurses (LPN); however, licensure is not required. Candidates with significant hospital, hospice, home health, or healthcare intake experience are encouraged to apply.

Preferred:

  • Current RN or LPN license (preferred but not required).
  • Experience in Home Health and/or Hospice Intake.
  • Familiarity with Medicare, Medicaid, commercial insurance plans, and regulatory admission requirements.
  • Experience collaborating with hospital case managers, social workers, physicians, and discharge planners.

Requirements

  • Minimum 5–7 years of healthcare intake, admissions, care coordination, discharge planning, or referral management experience.
  • Experience working in a hospital, hospice, home health, skilled nursing, rehabilitation, or healthcare setting.
  • Strong knowledge of medical terminology, patient care coordination, insurance verification, and healthcare documentation.
  • Excellent communication, organizational, and customer service skills.
  • Ability to prioritize multiple referrals and manage a high-volume workload.
  • Proficiency with Electronic Medical Records (EMR) systems and Microsoft Office applications.
