We are currently seeking qualified candidates for a full time Intake Coordinator position in the Houston area.
Job Description:
Primary function is to facilitate the intake referral process, coordinate care with the interdisciplinary team and the referral source.
Qualifications:
- Current Texas State license as a Licensed Vocational Nurse preferred
- Current Texas Drivers License
- One year experience as a Licensed Vocational Nurse, home health preferred
- Nursing skills as defined as generally accepted standards of practice
- Good interpersonal skills
- Proof of Hepatitis profile
- Experience with the intake process and procedures
- Experience in verifying insurance, Medicaid and Medicare
- Experience with Microsoft Office (Excel, Word) and Microsoft Outlook – required
- Detail oriented and highly organized
- Effective written and verbal communication
- Clean background and drug screen
- Reliable transportation
