Intake Coordinator
Location: Hewlett, NY
Job Type: Full-Time | Direct Hire
Schedule: Monday-Friday | Afternoon Shift
Compensation: $32/hour

A growing non-medical home care organization is seeking an experienced Intake Coordinator to join their operations team. This role will serve as the primary point of contact for new and existing clients, families, caregivers, and referral sources, ensuring a smooth intake process and timely coordination of services.

The ideal candidate will have prior experience within a home care or personal care agency environment and be comfortable managing client intakes, caregiver scheduling, insurance verification, Medicaid authorization processes, and high-volume communication.
Responsibilities:Client Intake & Coordination
  • Serve as the first point of contact for prospective and existing clients, families, referral sources, and caregivers.
  • Handle inbound calls and gather client demographic, service, and payer information.
  • Complete new client intake processes and ensure all required documentation is collected.
  • Enter and maintain accurate client information within scheduling and electronic record systems.
  • Communicate service details, timelines, and expectations with clients and families.
Scheduling & Caregiver Support
  • Coordinate caregiver schedules based on client needs, availability, and agency guidelines.
  • Manage schedule changes, cancellations, and call-offs.
  • Communicate updates, care preferences, and service information between clients, caregivers, and internal teams.
  • Assist with resolving scheduling conflicts and coverage gaps.
Insurance & Authorization Coordination
  • Verify insurance eligibility and authorization requirements for home care services.
  • Assist clients through Medicaid approval and authorization processes.
  • Obtain and track required documentation for insurance, private pay, and waiver services.
  • Communicate coverage details and service limitations to clients and internal staff.
Administrative Support
  • Maintain organized and accurate intake records.
  • Assist with audits, reports, and operational documentation.
  • Ensure intake procedures follow company policies and compliance standards.

Requirements:
  • 1-2+ years of experience as an Intake Coordinator, Care Coordinator, Scheduler, or similar role within a non-medical home care or personal care agency.
  • Experience handling client intakes and caregiver scheduling.
  • Knowledge of non-skilled home care services and workflows.
  • Experience verifying insurance eligibility and authorizations.
  • Familiarity with Medicaid processes and MSO/HMO terminology.
  • Strong communication and customer service skills.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Strong attention to detail and organizational skills.
  • Experience with home care scheduling systems or electronic records preferred.
  • Bilingual skills are a plus but not required.

Schedule:
  • Monday-Friday
  • Preferred hours: 12:00 PM - 8:00 PM
  • Alternative schedule may be considered: 3:00 PM - 8:00 PM
  • First two weeks of training will be completed during standard business hours.

Work Environment:
  • Fully onsite position.
  • Office-based role with frequent phone and computer use.
  • Current location in Hewlett, NY, with future office location expected within approximately a 20-minute radius of West Hempstead, including areas such as Uniondale or Garden City.
