We are seeking a mission-driven Infectious Disease Provider to join a highly collaborative, integrated care team focused on improving outcomes for patients living with HIV and other chronic infectious diseases, along with HIV prevention and care services for affected communities.

The Role

As an Infectious Disease Provider, you will:

  • Deliver comprehensive outpatient care for patients with complex chronic conditions
  • Diagnose and treat acute and chronic infectious diseases, including HIV
  • Share expertise in HIV prevention, STI prevention/care, and sexual health
  • Provide patient education and counseling to support long-term health outcomes
  • Collaborate across disciplines to design and implement holistic care plans
  • Participate in multidisciplinary case discussions to continuously improve care delivery

 

Ideal Candidate Profile

We are looking for providers who are:

  • Board-certified MD or DO / APP (Infectious Disease training strongly preferred)
  • Passionate about chronic disease management and population health
  • Comfortable working with Medicare, Medicaid, and diverse patient populations
  • Energized by team-based, collaborative care models
  • Committed to health equity and serving underserved communities
