Independent Housing Specialist

Reports to: Program Manager - MH IHP

Location: 6520 Basile Rowe, E Syracuse 13057

Pay Rate: $18.58 - $23.08 Hourly

Schedule: Monday - Friday: 9 a.m. - 5 p.m.

Our focus is supporting individuals by ways of residential housing, mental health and clinical services, and immersing our participants in community engagement. Working closely with the Office for People with Developmental Disabilities and the Office of Mental Health, we support 27 housing locations and 3 administrative buildings.?

About the Role: We are looking for a proactive employee to provide necessary support to eligible persons that promote their success at the highest level of independence possible.


Responsibilities:

  • Develop supportive relationships with individuals and get to know their unique capabilities, personality, and potential
  • Maintain a person-centered approach, and support individual preference and choice, to the extent possible under program guidelines
  • Support individuals by providing assistance with daily living skills and oversight, as needed
  • Assist participants with establishing and maintaining both mental and physical health care; promote independence in medication self-administration
  • Provide situational counseling, benefits, and entitlements, counseling
  • Engage in crisis intervention, as needed; facilitate the development of natural supports and encourage self-advocacy.
  • Assist Program Manager in developing a variety of housing options that may provide safe and affordable permanent housing to participants
  • Maintain ongoing relationships with community residence staff to support planning with individuals interested in exploring independent housing options
  • Possess a broad knowledge of community services which can be made available to participants, family members, service providers, and other significant supports
  • Assist in the referral processes for Agency and community programs; ensure responses to referrals are conducted in a timely manner, coordinate services with programs to identify participants’ areas of need and delegate supports appropriately
  • Complete documentation based on program requirements following established guidelines and timeframes
  • Maintain appropriate service, statistical, and financial records, as required, for coordination of services with other team members in a manner which respects confidentiality
  • Perform Other Duties as Assigned

Education/Experience:

Associate degree, plus 3 years of human services experience required. Bachelor’s degree, preferred. Will consider equivalent combination of relevant education and experience. Must have computer experience for documentation services.

Conditions of Employment:

Must have a valid New York State driver’s license in accordance with the Agency Vehicle and Driving policy and transportation necessary to meet program needs.
