member is seeking a compassionate caregiver to join his team The right
candidates must have some experience, comprehension, reliability,
ability to follow directions, and the ability to provide levels of
support to the member. This position is a great way to make the life of
a member easier by providing companionship and personal assistance in
Somerset, WI. Tasks include but are not limited to: Shower/Dressing
Assistance and hygiene assistance Transfer assistance using a
sit-to-stand Feeding and providing assistance with liquids Toileting
assistance Meal Assistance Light Housekeeping Companionship Schedule: 1.
Part-Time AM and PM Shifts 2. Day shift 9 a.m. to 1 p.m. and Evening
shift is 8 p.m. to 9 p.m. 4. Sunday through Saturday shifts available.
Wage: Wage: Pay is $20.00 per hour. Requirements: Must pass a criminal
background check. Valid driver's license and reliable transportation
Some experience is required. Who is Community Horizons Community
Horizons provides support to individuals and families enrolled in
self-directed care programs across Wisconsin, including services through
Managed Care Organizations, IRIS (Include, Respect, I Self-Direct), and
the Children's Long-Term Support (CLTS) program. Self-direction allows
participants to make their own choicesselecting who provides their care
and how those services are delivered. While Community Horizons offers
guidance and mentorship, we are not the employer of record. Participants
and families make all hiring decisions, with a third-party fiscal agent
