DESCRIPTION
The County of Yolo Sheriffs Office is recruiting to fill one ( 1 ) regular, Limited-Term position in the class of In-Custody Program Coordinator CalAIM.
The Yolo County Sheriffs Office is seeking a highly motivated and innovative professional to serve as the Sheriff's Offices first CalAIM In-Custody Program Coordinator. This position will lead the implementation, coordination, and evaluation of Californias CalAIM Justice-Involved initiative within the Yolo County Jail.
The Program Coordinator will oversee healthcare, behavioral health, and community support services provided to eligible incarcerated individuals during qualifying periods of custody. Working within a secure correctional environment, this position serves as the primary liaison between custody staff, healthcare providers, managed care plans, community-based organizations, and county partners to ensure compliance with CalAIM requirements and continuity of care.
This position is based primarily within a locked detention facility and requires regular interaction with incarcerated individuals, including those with complex medical, behavioral health, substance use, and social service needs. The successful candidate must be comfortable working in a correctional setting, building collaborative partnerships, navigating complex systems, and developing innovative solutions that improve outcomes for justice-involved individuals.
This is a unique opportunity to help build and shape a new county program that improves healthcare access, care coordination, and successful community reintegration for incarcerated individuals.
NOTE: The Limited-Term position will run for a 2 year period
Ideal Candidate
The ideal candidate is a collaborative and results-oriented professional who is passionate about improving outcomes for justice-involved populations and comfortable working within a correctional environment.
The candidate will have:
Experience administering healthcare, behavioral health, social service, reentry, or correctional programs serving vulnerable populations;
The ability to work effectively within a secure detention facility and interact professionally with incarcerated individuals, custody staff, healthcare providers, and community partners;
Knowledge of Medi-Cal, CalAIM, behavioral health, care coordination, case management, or related public-sector programs;
Strong project management and analytical skills, including the ability to develop programs, establish workflows, monitor performance measures, and evaluate outcomes.
Exceptional communication and relationship-building skills with the ability to collaborate across diverse agencies and stakeholder groups;
The ability to work independently, adapt to changing priorities, and navigate complex operational environments.
The strongest candidates will understand the unique challenges of delivering services within a jail setting and will be motivated by the opportunity to improve health outcomes, support successful transitions to the community, and reduce recidivism.
DEFINITION
Under general direction, coordinates, plans, develops, and evaluates the functions as it relates to the California Advancing and Innovating Medi-Cal (CalAIM) Program within a locked facility with incarcerated individuals and in the community; oversees; ensures program is evidence-based, evaluated for effectiveness through pre-determined performance measures, and in compliance with applicable policies and procedures, statutes, standards, legal mandates, and court orders; leads program development, data collection, and grant writing; and establishes partnerships with community organizations, county departments, law enforcement agencies, and other legal organizations to coordinate and enhance transition opportunities for CalAIM in-custody clients.
DISTINGUISHING CHARACTERISTICS
The CalAIM In-Custody Program Coordinator is a single-position class responsible for coordinating and administering a variety of programs and services related to CalAIM. Incumbents exercise independent judgment in planning and organizing program services. This class is allocated only to the Sheriff's Office and reports to a Correctional Lieutenant or higher-level position. Additionally, this position supervises professional and program-level staff and supports the Sheriff's Office in its strategic efforts to expand the use and availability of evidence-based programming and services to qualified CalAIM in-custody clients.
This class is distinguished from the In-Custody Program Coordinator in that the latter directs programs and services related to rehabilitation and recidivism reduction for inmates and offenders sentenced in Yolo County, whereas the CalAIM In-Custody Program Coordinator coordinates programs and services specific to the CalAIM program.

ESSENTIAL FUNCTIONS
Duties may include, but are not limited to, the following:
MINIMUM QUALIFICATIONS
Education: Bachelor's degree from an accredited college or university, preferably with a major in Business Administration, Criminal Justice, Behavioral/Social Sciences, Social Work, or related field; AND
Experience: Five (5) years of professional-level experience administering the delivery of health, mental health, and/or social services programs for in-custody or underserved and/or disadvantaged clients.
Substitution: A masters degree in a related field may substitute for one (1) year of experience.
License: This class requires the incumbent to possess and maintain a valid California drivers license, Class C or higher, to carry out job-related duties. Individual who does not meet this requirement due to a physical or mental disability may request reasonable accommodation.

Acceptable forms of educational documentation are official or unofficial transcripts that list the students name, institution name, field of study and date the degree was awarded.
For recent graduates who have not received proof of their degree yet, a letter from the registrars office stating that all requirements for graduation have been met and listing the field of study will be sufficient proof of graduation.
