- Shared call among all providers; RN call center handles routine calls
- Manageable patient census of 10 - 14 patients per day
- Focus on value-based care for complex geriatric patients aged 65 and older
- Experience with HEDIS measures and HCC coding highly valued
- eClinicalWorks EMR experience preferred
- Board-certified or board-eligible; DEA required
- Hospital-employed position with excellent work-life balance
