Description
CSPNJ, a mental health nonprofit agency, is looking for a Housing Supports Worker in Atlantic County. The Housing Supports Worker provides pre-tenancy and tenancy sustaining geared toward enabling individuals to live independently and work toward realizing their full potential.
HIGHLIGHTS
- Document observations, information, and services provided to the individuals in touchpoint progress notes, or any other emergent documentation as needed.
- Follows team protocol for charting and recording current treatment information for residents (i.e., ongoing documentation of current medications and follow-up appointments).
- Maintaining necessary casework records to document the provision of case management services for Medicaid reimbursement.
- Assess resident's daily living skills. Based on assessment, teach individuals skills in the areas of budgeting, cooking, laundry, cleaning, shopping, personal hygiene, and self-advocacy, as needed.
- Works with individual's to develop Housing Stabilization Plans and regularly review Level of Need Assessments
- Coordinate and manage services provided to residents.
Schedule: Monday-Friday: 8:00am-4:30pm | 40 hours per week | Full-time |
Benefit Eligible (Medical, free DMO dental & free vision) including 6 weeks PTO & 11 Holidays
Hourly Rate: $24.04/hour
Requirements
- High School Diploma/ GED plus 2 plus years of related work/life experience.
- The ability to write clear, concise, and accurate billable progress notes.
- Exceptional time management skills to be able to support the needs of multiple residents.
- Excellent communication, assessment, and people skills.
- A prominent level of patience and emotional resilience.
- The ability to remain calm under pressure when dealing with challenging situations.
- Empathetic with good listening skills.
- Great critical thinking skills and the ability to adapt and act accordingly to situations.
- Ability to carry out domestic duties.
- Ability to work with individuals experiencing homeless, mental health or substance use issues, and/or physical disabilities in a caring and professional manner.
- Proficient in Excel, MS Office products, as well as general computer literacy.
- Bilingual skills are a plus.
- Valid NJ Driver's License with acceptable driving record.
- Lived Experience: CSPNJ prioritizes hiring people who use their own life experience dealing with mental health/substance use issues to inspire and support others.
