Position Summary:
Vita Nova is seeking a compassionate and resourceful Housing Case Manager to support implement and administer its Permanent Supportive Housing (PSH) program for young adults with documented disabilities who are experiencing homelessness. Using a trauma-informed approach, the Housing Case Manager partners with program participants to develop individualized case plans, provide intensive housing-focused case management and life skills training, coordinate community resources, and provide support for the achievement of housing stability, improved social and emotional well-being, and pathways toward self-sufficiency.
This position works both in the office and throughout the community, and includes occasional evening, weekend, and rotational on-call responsibilities.
Key Responsibilities:
Provide intensive case management and develop individualized service plans focused on housing stability and goal achievement
Facilitate individual and group life skills training to promote self-sufficiency
Connect youth to community resources, including physical and mental healthcare, substance abuse services, education, employment, and social supports
Maintain accurate client documentation, case notes, and program data within HMIS and agency systems
Conduct assessments, monitor participant progress, and support crisis intervention and safety planning
Provide transportation to appointments and community-based services as needed
Participate in community partnerships and outreach efforts supporting homeless and disconnected youth
Qualifications:
Bachelor's degree in Social Work, Human Services, or related field required
Experience working with individuals experiencing homelessness and young adults with disabilities, including mental health and substance use disorders, preferred
Knowledge of Trauma-Informed Care and Positive Youth Development practices
Ability to identify community resources and assist youth with housing, employment, and life skills development
Strong crisis intervention, case management, documentation, and organizational skills
Proficiency in Microsoft Office Suite and general computer literacy
Excellent written and verbal communication skills
Ability to work independently and collaboratively in fast-paced community setting
Working knowledge of Microsoft Office and ability to document services in HMIS
Reliable transportation with a valid FL driver's license and ability to join company insurance policy
Ability to pass drug screen and a Level 2 background check through DCF Clearinghouse: https://info.flclearinghouse.com(Existing DCF Exemption Waivers will be considered)
