Opening for a Home Care Nurse Liaison-Discharge planner based at one Baltimore, MD hospital!! Looking for an MD RN with experience doing discharge planning in a hospital or home care setting.
Requirements for RN Case Manager/ Home Care Liaison:
- MD RN or compact RN license
- Previous case management/discharge planning experience required in a hospital or home care setting
- Home care experience preferred
- EPIC experience preferred
- MUST be fully vaccinated for COVID
Hospital Nurse Discharge Planner / Liaison Position Objective:
- The Home Care Nurse Liaison is a licensed nurse who has a thorough understanding of available home based services and products, eligibility requirements, and resources available in the community.
- The discharge planner verifies that the patient has the devices needed for nursing and home care such as walkers, commodes, etc.
- RN will also set up home care services if needed and advise of appropriate support organizations in the community.
- The position acts as a discharge planning resource to Hospital staff involved in home care planning, and facilitates continuity of care for the patient transitioning to home based services.
- Serves as on-site home-based care expert.
- The Home Care Nurse Liaison acts as a resource to Hospital/Facility staff involved in home care planning, and facilitates continuity of care for the patient transitions to home based services. The Liaison is actively involved in marketing activities and the presentation of educational programs to build revenue and promote the continuum of care needs for patients.
- Thorough knowledge of home care (home health, infusion, and medical respiratory and equipment) eligibility requirements, agency policy and procedures, federal and state regulations and third party regulations for reimbursement of home care services.
Hours: M-F , 8:30am-5pm
- Weekend:there may be a weekend need on Saturday and Sunday 8:30am – 4pm
