Position Summary:
 
The Hospital Coordinator serves as the primary operational liaison between hospital-based providers, hospital support staff, post-acute partners, and administrative teams. This role is responsible for coordinating daily provider operations, managing patient tracking and assignments, supporting transitions of care, facilitating communication across the continuum, and ensuring accurate documentation, billing, payroll submission, and scheduling activities.
 
Essential Duties and Responsibilities:
 
Provider Operations & Scheduling
  • Coordinate and maintain provider coverage schedules.
  • Schedule provider meetings, operational reviews, and leadership meetings.
  • Coordinate C-suite meetings, Medical Operations Reviews (MORs), and other administrative meetings.
  • Serve as a liaison between hospital support staff, providers, and operational leadership.
  • Support provider onboarding and ongoing operational needs.
Patient Assignment & Tracking
  • Track hospitalized patients and assign providers based on payer relationships, PCP affiliations, and organizational guidelines.
  • Monitor patient census and maintain accurate patient assignment records.
  • Verify that all assigned patients have been seen by the appropriate provider.
  • Follow up on outstanding patient encounters and rounding activities.
  • Track patient discharge orders and anticipated discharge dates.
  • Monitor patient movement throughout the hospital stay to ensure continuity of care.
Care Coordination & FastPAC Support
  • Facilitate patient handoff to the FastPAC program and post-acute care teams.
  • Coordinate communication between hospital providers, skilled nursing facilities, PCPs, care managers, and other stakeholders.
  • Support transition planning activities and ensure appropriate information is communicated during care transitions.
  • Schedule PCP follow-up appointments upon discharge when appropriate.
  • Assist with care coordination initiatives designed to improve patient outcomes and reduce readmissions.
Clinical Operations Support
  • Attend interdisciplinary rounds two to three times per week.
  • Track action items and follow-up needs identified during rounds.
  • Communicate patient status updates and discharge planning information to appropriate stakeholders.
  • Assist providers with operational workflow management and issue resolution.
Documentation, Billing & Payroll Support
  • Verify completion of provider documentation and encounter requirements.
  • Support billing workflow by ensuring documentation is complete and submitted timely.
  • Assist with payroll submission processes and provider productivity tracking.
  • Maintain accurate operational reports and performance metrics.
  • Support audits and compliance initiatives as needed.
Requirements:
  • Associate's degree; bachelor's degree in healthcare administration or over 8 years of successful performance and meeting below requirements.
  • 2–5 years of healthcare operations/project coordination and organization, care coordination, physician practice management, hospital administration, and proficiency in managing multiple priorities in a fast-paced healthcare environment with strong attention to detail and follow-through.
  • At least 2 years of experience working within hospital, post-acute, or physician practice environments with knowledge of healthcare workflows, patient transitions, and provider operations with proven track record of building successful relationships with physicians, hospital staff, SNF partners, and community providers.
  • At least 2 years' experience and proficiency with EMRs, Microsoft Office Suite, and scheduling systems.
  • Excellent verbal and written communication abilities.
  • Knowledge of transitions of care, discharge planning, and value-based care initiatives preferred.
