Job Requirements
  • Education Requirements: Valid RN license in the state of Wisconsin
  • Certifications Requirements: BLS certification, Valid Driver’s License, Auto Insurance, Vehicle registration
  • Experience Requirements: Minimum of 2 years hospice experience; Prior experience with EPIC EMR preferred; Strong communication skills in end-of-life care
  • Other Requirements: Ability to work independently, excellent interpersonal skills, comfort with end-of-life medications and symptom management

Job Responsibilities
  • Provide compassionate end-of-life care to hospice patients within their homes, SNF, or assisted living facilities, ensuring comfort and dignity.
  • Collaborate with physicians, case managers, and liaisons to coordinate patient care and develop personalized care plans.
  • Administer medications and symptom management interventions, including opioids and other end-of-life medications, following best practices and safety protocols.
  • Complete thorough patient assessments and documentation using EPIC EMR, ensuring accuracy and compliance.
  • Educate patients, families, and healthcare providers about hospice services, expectations, and medication management.
