What You'll Do
- Lead and support the daily clinical operations of our hospice nursing team.
- Mentor, coach, and develop nurses while fostering a collaborative, patient-centered culture.
- Ensure compliance with Medicare Conditions of Participation, state regulations, and hospice best practices.
- Oversee clinical documentation, patient admissions, care planning, and quality initiatives.
- Coordinate interdisciplinary team meetings and strengthen communication across nursing, social work, chaplaincy, hospice aides, and providers.
- Support performance improvement, regulatory readiness, audits, and quality outcomes.
- Partner with executive leadership to continuously improve clinical operations and patient care.
What We're Looking For
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience required.
- Previous Clinical Supervisor, Clinical Manager, Director of Clinical Services, Patient Care Manager, or other nursing leadership experience preferred.
- Strong knowledge of hospice regulations, Medicare Conditions of Participation, and quality standards.
- Excellent communication, coaching, and problem-solving skills.
- A collaborative leader who builds trust, develops people, and leads by example.
