Build and maintain trusted relationships with physicians, hospital staff, and care teams through daily in‑person collaboration and communication.
Serve as the primary clinical resource for hospice education, eligibility, and appropriateness.
Conduct patient assessments and coordinate the admission process.
Partner with Admissions, Clinical, and Care Teams for smooth transitions.
Advocate for patients and families during goals‑of‑care conversations.
Participate in interdisciplinary planning, training, and outreach.
Uphold ethics, quality, and continuous learning.
Meet expectations for response times, referral coordination, and communication.

If you meet these qualifications, we want to meet you!
One year of RN experience with knowledge of home health regulations, Medicare requirements, and care‑plan development
Medical sales or clinical outreach experience preferred
Strong communication and relationship‑building skills
Ability to work independently in a hospital environment
Passion for serving with dignity and empathy
Active RN license
Valid driver’s license and insured vehicle
