Job Details

Hospice RN Case Manager – Queens, NY (Field-Based)

Seeking a Registered Nurse / RN Case Manager to manage a hospice caseload across Queens in home, SNF, and inpatient settings.

This role focuses on providing end-of-life care, coordinating services, and ensuring patient comfort through structured care planning and interdisciplinary collaboration.

Key Responsibilities:

Manage hospice caseload (home + facility-based patients)
Develop and implement individualized care plans
Perform physical + psychosocial patient assessments
Coordinate care with LPNs, HHAs, caregivers, and facilities
Ensure proper equipment, supplies, and care delivery
Maintain EMR documentation and compliance standards

Schedule:

Monday–Friday
8:30 AM – 5:00 PM
No weekends

Territory:

Queens (field-based)
