What You'll Do
As a Hospice RN Case Manager, you will serve as the primary nurse coordinating care for patients and supporting families through every stage of the hospice journey.
Clinical Responsibilities
- Manage a caseload of hospice patients in private homes, assisted living communities, and skilled nursing facilities
- Perform comprehensive nursing assessments and ongoing evaluations
- Develop, implement, and update individualized plans of care
- Manage pain and symptom control to maximize patient comfort and quality of life
- Provide education and emotional support to patients and their families regarding disease progression and end-of-life care
- Coordinate care with physicians, social workers, chaplains, CNAs, Home Health Aides, and the hospice interdisciplinary team
- Supervise Certified Nursing Assistants (CNAs) and Home Health Aides (HHAs)
- Maintain accurate and timely documentation within the Electronic Medical Record (EMR)
- Ensure compliance with Medicare, state, and organizational hospice regulations
Qualifications
Required
- Active North Carolina Registered Nurse (RN) license
- Current CPR certification
- Valid driver's license
- Reliable transportation with current automobile insurance
- Strong assessment, communication, and time management skills
Preferred
- Minimum of one year of RN experience
- Hospice experience
- Home Health experience
- Palliative Care experience
- Case Management experience
- Experience using Electronic Medical Records (EMR)
