Reporting to the Managing Director – Hospice, the RN Case Manager, provides skilled nursing care to hospice patients in their homes while coordinating individualized care plans with patients, families, and the interdisciplinary team (IDT). You'll serve as a trusted advocate, ensuring compassionate, high -quality care throughout the hospice journey.<\/span><\/span>

What You’ll Do<\/span><\/span>

Key Responsibilities<\/span><\/span>
<\/p>

• Perform comprehensive patient assessments and develop individualized care plans<\/span><\/span>
  <\/li>
• Deliver skilled nursing care in patients homes in accordance with hospice standards<\/span><\/span>
  <\/li>
• Coordinate care with patients, families, physicians, and the IDT<\/span><\/span>
  <\/li>
• Supervise Hospice Aides and LVNs to ensure continuity of care<\/span><\/span>
  <\/li>
• Maintain accurate, compliant clinical documentation<\/span><\/span>
  <\/li>
• Complete patient admissions and participate in PRN on -call rotation<\/span><\/span>
  <\/li>
• Manage patient schedules and prioritize changing care needs<\/span><\/span>
  <\/li>
• Promote infection control, patient advocacy, and agency values<\/span><\/span>
  <\/li>
• Assist with training and mentoring new clinical staff as needed<\/span><\/span>
  <\/li>
• Commit to ongoing professional growth and clinical excellence through continuous education<\/span><\/span>

  What We Are Looking For<\/span><\/span>
  <\/p>

  Qualifications<\/span><\/span>
  <\/p>

  • Graduate of an accredited Nursing program (Diploma, ADN, or BSN)<\/span><\/span>
    <\/li>
  • Active Texas RN license required; Current CPR certification<\/span><\/span>
    <\/li>
  • 1–3 years of clinical nursing experience required; Hospice, ER, or OR experience preferred<\/span><\/span>
    <\/li>
  • Reliable transportation, valid Texas driver's license, and current auto insurance<\/span><\/span>
    <\/li>
  • Must pass background check and provide required TB and Hepatitis documentation<\/span><\/span>
    <\/li><\/ul>

    • Ability to stand, walk, safely move patients, lift up to 50 lbs. and travel locally<\/span><\/span>
      <\/span><\/li>
    • Ability to work effectively in high -pressure, fast -paced environments<\/span><\/span>
      <\/span><\/li>
    • Strong clinical judgment and quick decision -making skills<\/span><\/span>
      <\/span><\/li>
    • Ability to meet the individualized psychosocial needs of patients and families<\/span><\/span>
      <\/span><\/li>
    • Excellent hand -eye coordination and manual dexterity<\/span><\/span>
      <\/span><\/li><\/ul>
