We are seeking a RN to deliver skilled hospice care, communicate effectively with patients and families, and make a meaningful difference in every visit.

Provide skilled nursing care and manage symptoms in the patient’s home
Develop individualized care plans with the interdisciplinary team
Communicate and educate patients and families
Document care accurately and efficiently
Participate in shared on-call responsibilities
Current Arizona RN license
At least 1 year of nursing experience (hospice/palliative care preferred)
Associate’s or Bachelor’s degree in Nursing
CPR certification, valid driver’s license, reliable vehicle, and insurance
Compassion, strong clinical judgment, and ability to work independently
