What You'll Do
- Manage an assigned hospice caseload throughout the continuum of care.
- Provide expert pain and symptom management focused on comfort and quality of life.
- Serve as the primary Hospice RN Case Manager for patients and families.
- Complete comprehensive nursing assessments and individualized care plans.
- Educate and support caregivers through every stage of the hospice journey.
- Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.
- Provide care in patients' homes, assisted living communities, and skilled nursing facilities.
- Maintain accurate EMR documentation and participate in a weekend and on-call rotation.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Experience in hospice preferred, or a strong background in oncology, ICU, palliative care, home health, or other complex clinical settings.
- Strong clinical judgment and the ability to work independently.
- Exceptional communication and relationship-building skills.
- A passion for patient-centered, compassionate hospice care.
- Reliable transportation and willingness to travel throughout your assigned territory.
