What You'll Do
- Serve as the primary Hospice RN Case Manager for your assigned patients and families.
- Complete comprehensive nursing assessments and develop individualized plans of care.
- Manage pain and symptoms while promoting comfort, dignity, and quality of life.
- Educate patients, caregivers, and families throughout the hospice journey.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and community partners.
- Respond to changing patient needs while providing compassionate guidance and clinical expertise.
- Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Two or more years of hospice experience preferred, or a strong background in oncology, palliative care, home health, ICU, or other complex care settings.
- Excellent assessment skills and independent clinical judgment.
- Strong communication and relationship-building abilities.
- Compassionate, patient-centered approach to end-of-life care.
- Exceptional organization, time management, and critical thinking skills.
- Reliable transportation and willingness to travel throughout your assigned territory.
