The Admissions Nurse is responsible for facilitating patient transitions of care during admission which includes coordination with Central Intake, Clinical Manager, Program Director and Community Representative. A qualified admissions nurse is a licensed RN that will provide visit support to new admissions under our hospice care. This role requires working Monday through Friday, from 9:00a.m. to 5:030p.m. Hours could change based off Admission needs.
