    We are partnering with a well-established, multi-state hospice provider known for delivering compassionate, patient-centered care through interdisciplinary teams in both home and facility settings.     Pay and Schedule

    $40–$46/hour

    • Monday–Friday, 8:00am–4:30pm
    • On-call rotation approximately every 5 weeks
    • Includes weekend coverage (Saturday and Sunday)
    • $6/hour hold rate plus time-and-a-half for visits during call

    Responsibilities

    • Manage hospice patient caseload across home and facility settings
    • Perform patient assessments and develop individualized care plans
    • Provide skilled nursing care and symptom management
    • Coordinate with interdisciplinary team members
    • Maintain documentation using Homecare Homebase
    • Participate in on-call rotation

    Territory

    • Coverage includes York and Lancaster areas
    • Patients are assigned geographically to support efficiency
    • Typical day includes approximately one hour of drive time
    • Mix of facility and home-based hospice care

    Requirements

    • Active RN license
    • At least one year of clinical experience (acute care, home health, or hospice preferred)
