What You'll Do
- Manage an assigned hospice caseload while serving as the primary nurse for patients and families.
- Complete comprehensive nursing assessments and develop individualized plans of care.
- Deliver expert pain and symptom management focused on comfort, dignity, and quality of life.
- Educate patients, caregivers, and families throughout every stage of the hospice journey.
- Coordinate care with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and other interdisciplinary team members.
- Provide care in private homes, assisted living communities, and skilled nursing facilities.
- Maintain accurate, timely EMR documentation while ensuring compliance with Medicare hospice regulations.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Experience in hospice, oncology, ICU, palliative care, home health, or another high-acuity clinical setting.
- Strong assessment skills, sound clinical judgment, and the ability to work independently.
- Passion for relationship-centered, patient-focused care.
- Excellent communication, organization, and critical thinking skills.
- Reliable transportation and willingness to travel throughout the service area.
