AccentCare is seeking an experienced physician for a combined Hospice and Palliative Care Medical Director role in Columbia, MD. This full-time position encompasses medical direction across both service lines and is designed for a clinician ready to drive clinical excellence, support an interdisciplinary care model, and build meaningful community relationships.

Be the Best Hospice Physician You Can Be
If you meet these qualifications, we would love to meet you:
- Must be a licensed Medical Doctor (M.D.) or Doctor of Osteopathy (D.O.) in the state(s) of practice, with training or experience in caring for chronically or terminally ill patients
- Maintains active DEA license
- Valid driver's license
- Requires strong clinical judgment, administrative capabilities, and excellent communication skills to support patient care and interdisciplinary collaboration
- Must demonstrate a clear commitment to AccentCare's philosophy, mission, values, and culture of compassionate end-of-life care
Responsibilities:
- Provide overall medical direction for both hospice and palliative care services across the Maryland program
- Supervise hospice team physicians and ensure 24/7 physician coverage for patient needs
- Partner with Executive Director and Director of Clinical Services on planning and evaluation for both service lines
- Serve on the Executive Team and lead strategic planning across both service lines
- Set and uphold standards for documentation, compliance, and quality metrics across both service lines
- Lead IDG meetings for individualized, anticipatory care planning
- Conduct in-person and virtual hospice visits with timely documentation
- Participate in call coverage rotation for after-hours and newly admitted patients
- Guide symptom management: pain, nausea, end-of-life comfort care
- Certify and recertify hospice eligibility per regulatory requirements
- Supervise nurse practitioners delivering palliative care in acute and post-acute settings
- Provide clinical oversight and sign-off on NP assessments, goals-of-care conversations, and care plans
- Serve as physician of record and escalation point for complex palliative cases
- Lead education for NPs, nursing staff, and community providers on serious illness communication and advance care planning
