Position Summary


We are seeking a compassionate and experienced Hospice Nurse Practitioner (NP) to join our interdisciplinary team. The NP will provide advanced clinical care to hospice patients, focusing on comfort, symptom management, and quality of life. This role works closely with physicians, nurses, social workers, and other care team members to ensure patient-centered care.


Key Responsibilities


  •  Provide comprehensive assessments, diagnosis, and treatment planning for hospice patients 
  •  Manage pain and symptoms in accordance with hospice and palliative care guidelines 
  •  Collaborate with the hospice interdisciplinary team to develop and update care plans 
  •  Conduct routine and urgent patient visits in homes, facilities, or hospice settings 
  •  Educate patients and families on disease progression, medications, and care plans 
  •  Document all patient interactions accurately and timely in EMR 
  •  Participate in IDG (Interdisciplinary Group) meetings 
  •  Ensure compliance with all regulatory and organizational standards 
Qualifications


  •  Current Nurse Practitioner license in the state of [Insert State] 
  •  Board certification as a Nurse Practitioner (FNP, AGNP, or related) 
  •  Minimum of 1–2 years of experience in hospice, palliative care, or related field preferred 
  •  DEA license (if prescribing controlled substances) 
  •  Strong clinical assessment and critical thinking skills 
  •  Excellent communication and interpersonal skills 
  •  Ability to work independently and as part of a team 
Preferred Qualifications


  •  Experience in hospice or palliative care
