As a Hospice Medical Social Worker, heres a big-picture view of what youll do:
Provide home-based social services to patients dealing with end-of-life issues including care management, pain, symptoms associated with chronic and terminal illnesses, depression, anxiety, poor nutrition, or a lack of resources that make managing their journey difficult.
Work as part of a hospice team which will include an RN, LPN, CNA, Spiritual Counselor, Volunteers, Hospice Aides, PT, OT, or SLP, focused on the patients plan of care and individual goals.
Monitor a patients psychosocial condition and identify social and emotional needs.
Conduct patient assessments, document progress, and ensure patient is moving toward goals.
Provide counseling, community resource planning, crisis intervention, and advocacy.
Consult with family and caregivers on the patients plan of care and how to help them progress.
A few must-haves for Home Health Medical Social Workers:
• Masters degree in Social Work and active MSW license in Ohio • Minimum of 2 years of social work experience, ideally in home healthcare • CPR certification - Can provide if needed • Strong interpersonal skills, good communicator, empathetic, compassionate, and resourceful
