Hospice Master Social Worker (MSW/LMSW/LCSW) - Full -Time

The Staff Pad is proud to partner with a leading hospice and palliative care provider in the DFW Metroplex,to hire a compassionate Master Social Worker (MSW/LMSW/LCSW).<\/span><\/span>
<\/p>

Reporting to the Director of Family Care Services, The Master Social Worker provides coordinated hospice care by conducting psychosocial assessments, counseling patients and families, coordinating community resources, and supporting emotional, financial, environmental, and end -of -life needs.<\/span><\/span>

What You'll Do<\/span><\/span>
<\/p>

Key Responsibilities<\/span><\/span>
<\/p>

• Conduct psychosocial assessments to identify patients' and families' emotional, social, financial, environmental, and resource needs<\/span><\/span>
  <\/li>
• Provide counseling and emotional support to patients and families navigating grief, loss, and end -of -life care<\/span><\/span>
  <\/li>
• Educate and facilitate advance care planning discussions with patients and families<\/span><\/span>
  <\/li>
• Coordinate community resources, referrals and support services to meet patient and family needs<\/span><\/span>
  <\/li>
• Develop and support individualized care plans<\/span><\/span>
  <\/li>
• Collaborate with physicians, nurses, chaplains, and the interdisciplinary team<\/span><\/span>
  <\/li>
• Maintain accurate and timely clinical documentation<\/span><\/span>
  <\/li>
• Communicate effectively with patients, families, and healthcare providers<\/span><\/span>
  <\/li>
• Participate in interdisciplinary team meetings and collaborate on patient care planning<\/span><\/span>
  <\/li>
• Maintain required licensure, continuing education, and professional competency<\/span><\/span>
  <\/li>
• Maintain knowledge of community resources and public/private assistance programs<\/span><\/span>
  <\/li>
• Manage clinical assignments within scope of education, licensure, and competency<\/span><\/span>
  <\/li>
• Promote a culture of compassionate, patient -centered care aligned with agency values and policies<\/span><\/span>

  Qualifications<\/span><\/span>
  <\/p>

  • Master's Degree in Social Work (MSW) required<\/span><\/span>
    <\/li>
  • Texas LMSW or LCSW license required<\/span><\/span>
    <\/li>
  • Minimum one year of healthcare social work experience<\/span><\/span>
    <\/li>
  • Hospice, palliative care, home health, or related healthcare experience preferred<\/span><\/span>
    <\/li>
  • Valid Texas driver's license, reliable transportation, and auto insurance required<\/span><\/span>
    <\/li><\/ul>
