    What You'll Do

    • Provide skilled nursing care including medication administration, wound care, symptom management, and patient monitoring.
    • Identify and communicate changes in patient condition to the RN Case Manager and interdisciplinary team.
    • Educate patients, caregivers, and families about medications, comfort measures, and hospice services.
    • Deliver care in homes, assisted living communities, memory care, and skilled nursing facilities.
    • Complete timely EMR documentation and participate in weekend, holiday, and on-call rotations.

    What You'll Bring

    • Active Minnesota Licensed Practical Nurse (LPN) license.
    • Previous experience in hospice, home health, long-term care, skilled nursing, oncology, or palliative care.
    • Excellent communication and interpersonal skills.
    • Ability to work independently while collaborating with the interdisciplinary team.
    • Compassion, professionalism, and strong clinical judgment.
