We're looking for a compassionate, skilled LPN to join our Hospice team in a full-time, field-based role serving patients and families throughout the Cleveland / Northeast Ohio area.
Compensation
- $26.00 – $33.00 per hour
- Expected 40 hours per week
- Additional coverage opportunities available in Home Health and Palliative Care

What You'll Do
- Provide direct patient care to hospice patients in their homes in accordance with the established plan of care
- Monitor and document patient symptoms, comfort levels, and changes in condition
- Administer medications and treatments as directed by the RN Case Manager or physician
- Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals
- Provide education and emotional support to patients and families navigating end-of-life care
- Maintain accurate and timely documentation in the EMR within required timeframes
- Provide coverage in Home Health and Palliative Care settings as needed
Requirements
Must-Have:
- Active LPN license (Ohio) + current BLS certification
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
- Minimum 2 years of clinical nursing experience
Preferred / Nice-to-Have:
- Prior hospice, palliative care, or home health experience
- Experience with end-of-life care and comfort-focused care delivery
- Familiarity with HCHB or similar home health/hospice EMR
- Strong communication and family support skills
