We're looking for a compassionate, skilled LPN to join our Hospice team in a full-time, field-based role serving patients and families throughout the Cleveland / Northeast Ohio area.


Compensation

  • $26.00 – $33.00 per hour
  • Expected 40 hours per week
  • Additional coverage opportunities available in Home Health and Palliative Care

What You'll Do

  • Provide direct patient care to hospice patients in their homes in accordance with the established plan of care
  • Monitor and document patient symptoms, comfort levels, and changes in condition
  • Administer medications and treatments as directed by the RN Case Manager or physician
  • Communicate effectively with patients, families, and the interdisciplinary care team regarding patient status and care goals
  • Provide education and emotional support to patients and families navigating end-of-life care
  • Maintain accurate and timely documentation in the EMR within required timeframes
  • Provide coverage in Home Health and Palliative Care settings as needed

Requirements

Must-Have:

  • Active LPN license (Ohio) + current BLS certification
  • Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
  • Minimum 2 years of clinical nursing experience

Preferred / Nice-to-Have:

  • Prior hospice, palliative care, or home health experience
  • Experience with end-of-life care and comfort-focused care delivery
  • Familiarity with HCHB or similar home health/hospice EMR
  • Strong communication and family support skills
