    Schedule:Part-Time to Start | Weekend, On-Call, and Holiday Rotation

    What You'll Do

    • Provide skilled hospice nursing care including medication administration, symptom management, wound care, and patient monitoring.
    • Observe changes in patient condition and communicate with the RN Case Manager and interdisciplinary team.
    • Educate patients and caregivers while providing compassionate, relationship-centered care.
    • Deliver services in private homes, assisted living communities, memory care, and skilled nursing facilities.
    • Maintain accurate EMR documentation.
    • Participate in weekend, holiday, and on-call rotations as scheduled.

    What You'll Bring

    • Active Minnesota Licensed Practical Nurse (LPN) license.
    • Previous experience in hospice, home health, long-term care, skilled nursing, oncology, or palliative care.
    • Strong communication and clinical assessment skills.
    • Ability to work independently while collaborating with an interdisciplinary team.
    • A compassionate, dependable, and patient-focused approach to care.
