Your Impact: Key Responsibilities

You are the vital engine ensuring timely and seamless patient admissions. Your core responsibilities include:

  • First Point of Contact: Professionally manage all incoming calls and swiftly process referrals.
  • Admission Coordination: Efficiently coordinate and schedule patient evaluations. FinancialRegulatory Management: Verify all Medicare, Medicaid, and private insurance benefits, including securing necessary authorizations.
  • DocumentationData Integrity: Create, manage, and maintain accurate electronic medical records (EMR) and assemble all patient medical records and admission documents with precision.
  • Seamless Communication: Ensure smooth coordination by communicating proactively with referral sources and community partners.
  • Operational Support: Provide essential clerical and administrative support to maintain

seamless organizational operations.

  • Compliance: Uphold full compliance with all Medicare, State, Federal, and accreditation standards.

Qualifications

Required:

  • High School Diploma or equivalent
  • Proven experience in medical records or healthcare administration
  • Exceptional organizational skills and meticulous attention to detail
  • Strong verbal and interpersonal communication skills
  • A professional, dependable, and team-oriented mindset
  • Current CPR certification

Preferred:

  • Direct Hospice Intake or home health experience
