Description:
Provides care ordered by the physician as indicated by the plan of care to all patients on caseload including baths, oral hygiene, nail care, foot care, skin care, pressure relief, dressing patient, feeding, and transferring patient.
Takes accurate vital signs.
Describes the activities of daily living and demonstrates the ability to use techniques to assist the patient to meet these activities.
Participates in case conferences (IDG's) to discuss multidisciplinary team responsibilities, patient progress, plans for continued care, new problems, etc.
Notifies appropriate licensed staff when patient complains of pain.
Positions patient in bed with proper body alignment.
Assists patient with ambulation as necessary.
Performs aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
Supports cultural practices as long as these practices do not harm the patient/family or interfere with the planned course of medical treatment.
Identifies and communicates findings to the Case Manager and/or Clinical Manager.
Participates in the Agency's other mandated activities, as requested by Clinical Manager or Director of Patient Care Services (DPCS).
Provides clear, concise documentation in medical record as it relates to reimbursement guidelines and Agency policy and procedure.
Identifies and communicates findings to the supervising RN.
