Home Care / Hospice Coordinator
Make a Meaningful Impact in Patient Care Transitions
We are seeking a Home Care / Hospice Coordinator to serve as a key liaison between the acute care setting and post‑acute home care and hospice services. In this role, you will help ensure smooth, timely, and compassionate transitions for patients and families while building strong relationships with hospital staff and community providers.
This is an excellent opportunity for a clinically experienced professional who enjoys collaboration, education, and patient advocacy-while also contributing to referral growth and service excellence.
What You'll Do
- Serve as the primary point of contact for hospital case managers, discharge planners, physicians, and care teams regarding home care and hospice services
- Facilitate and coordinate referrals from the acute care setting to home care or hospice
- Educate hospital staff, physicians, patients, and families on available services and care options
- Conduct initial assessments of home care needs and document care plans in compliance with policy
- Obtain physician orders and coordinate timely initiation of services
- Gather, review, and transmit clinical documentation to agency intake
- Communicate insurance coverage details and explain financial responsibility to patients and families
- Maintain ongoing communication with physicians, intake teams, families, and care partners
- Participate in quality improvement initiatives and support new program development
- Build and maintain referral relationships through hospital rounding and provider visits
- Meet established referral or sales performance expectations
- Uphold confidentiality, compliance standards, and organizational values at all times
What We're Looking For
Required Qualifications
Bachelor's degree or equivalent experience
Minimum of 2 years of experience in acute care, care coordination, customer service, sales/marketing, or a related role
Excellent verbal and written communication skills
Valid driver's license and reliable transportation
Preferred Qualifications
- Graduate of an approved clinical education program
- Current clinical license in the state of employment
- Experience with discharge planning, case management, or post‑acute care services
