Job Summary and Responsibilities

As our Hospice Aide, you will provide compassionate and essential direct care to our patients, significantly enhancing their comfort and quality of life during a critical time. You will be a vital source of support for both patients and their families.

Every day you will assist patients with personal care activities such as bathing, dressing, and mobility, while also providing light household tasks and companionship. You will be expected to observe and report changes in patient condition, maintain a clean and safe environment, and offer emotional support in a dignified and respectful manner, all under the supervision of a registered nurse.

To be successful in this role, you will need to possess a deeply empathetic and patient-centered approach, strong communication skills, and a commitment to providing respectful and high-quality care, coupled with relevant certification as a Certified Nursing Assistant (CNA).

  • Implements personal care and assistance in the delivery of direct and indirect patient care while following the Hospice Aide care plan
  • Performs documentation that is timely, accurate, and concise
  • Assesses patient condition and status every visit and communicates observation and change to Registered Nurse, Physical Therapist, Occupational Therapist, or Home Health Clinical/Office Coordinator
  • Performs efficiently in emergency situations, following established protocols, appropriate communication and documentation within Hospice Aide practice/limits
  • Performs personal care services to and for patients in their homes
  • Assists in determining patient's needs and communicates need to case manager

Job Requirements

Required

  • High School Graduate or GED
  • Current Home Health Aide Certificate issued by the California Department of Health Services
  • One year recent work experience
  • Home Health Aide: CA
  • American Heart Association Basic Life Support - CPR
  • Driver's License: CA
  • Auto Insurance

Preferred

  • Experience as a Home Health Aide
