Optimal Care is growing and proactively building our talent pipeline for future opportunities. While this role may not be immediately available, we invite you to submit your resume to this posting. A member of our Recruiting team will reach out to discuss opportunities with you.

Optimal Care is where your dedication meets a rewarding career.

As a clinician-owned and operated organization, we empower our team members to provide personalized, compassionate care to patients and families every day.

For 6 consecutive years, we have earned the Great Places to Work certification, and we believe that exceptional care starts with exceptional people. We're committed to supporting your professional growth, valuing your expertise, and creating an environment where you can do your best work every day.

We are a leading provider of Physician Services, Home Health, and Hospice that encourages collaboration, innovation, and clinical excellence. From chronic disease management and rehabilitation to end-of-life care, our focus is on improving outcomes and enhancing quality of life.

We live a simple Mission: Serve Together, Provide Value, and Deliver Exceptional Quality Care.

Why Join Us as an After-Hours Registered Nurse?

  • Work-life balance: Start and finish your shift at home, working on-call for 7 consecutive nights (5 PM – 8 AM, Monday–Sunday), then enjoy the next 7 days completely off. That's just 26 weeks of work per year—with a full-time salary—giving you half the year off while maintaining full-time pay.
  • No extra on-call surprises: Your schedule is set, so you can confidently plan your personal life without unexpected obligations.
  • Optimized territories: Our territories are designed to minimize drive times while still meeting the urgent needs of our patients, making your work more efficient and focused.

What You'll Do:

  • Deliver urgent patient care after hours, including admissions, symptom management, and other pressing needs. Think of this role as the "emergency room" for hospice patients in your neighborhood.
  • Be a reassuring presence for patients, families, and our care team, providing clear, compassionate communication with physicians, nurse practitioners, and hospice case managers.

What You Won't Do:

  • You won't be responsible for triaging patients on your own—our triage team handles that and dispatches visits only when an in-person RN is required.
  • You won't manage an assigned caseload or ongoing patient care. Your focus is providing immediate, high-impact support when it matters most.

  • Graduate of an accredited Diploma, Associate, Baccalaureate, or Masters Degree in Nursing.
  • Current State License as a Registered Nurse
  • Ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment

  • Ability to critical think and prioritize the needs of multiple patients.
  • Reliable transportation and valid automobile insurance coverage
  • Ability to sit, stand, bend, lift, and move intermittently
  • Ability to lift at least 35 lbs. and bear the weight of an average adult effectively
